Work Permit Form

Work Permit: Steps to Obtain 2025-26


Must have a job offer to obtain a work permit which is work site specific and mandatory until your 18th birthday.


Please see me in my main gym office before school starts or after school ends. Tuesdays I’m in 154 for work experience before school. Nutrition or lunch are available by appointment. Please email to check my availability. Please don’t leave a permit application in my mail box in the main office.

  1. Get a copy of Intent to Employ a Minor form. It can be obtained from Mr. Hunt in the gym before school, googled, downloaded from the LAUSD website, or click the link below to an editable PDF.

  1. Fill out the minor’s information and school information.
  2. Bring form to your employer to be and have them complete the employer section including basic    description, printed name, signed name, and dated.
  3. Bring form to parent or guardian for a signature.
  4. The form must be filled out completely and signed with all “wet” signatures.
  5. Return form to Mr. Hunt in the gym, before school or after school. Place nothing in my mailbox.
  6. Bring proof of date of birth (Birth Certificate, Passport, CA ID, or CA Driver’s License).
  7. Permit ready for pickup up in no more than 48 hours after the completed application is submitted and accepted. I can generally complete it by the next day. Please confirm with Mr. Hunt.

When school isn’t in session during winter (12/15/25 - 1/9/26), spring (3/30/26 - 4/4/26), or summer (6/15/26 - 8/14/26.

  5. Call the Work Experience Office (LAUSD Headquarters) – 333 South Beaudry Ave. LA 90017, entrance is on 4th street off Boylston, at (213) 241-7053 or use the website to confirm hours.

  7. You’ll need a letter of authorization from Venice HS if the request is during school. I’ll create that letter.
  8. This letter isn’t required for a summer permit when grades and attendance isn’t a factor. 

Minors are required to:

  1. Maintain satisfactory school attendance (90% +) with minimal tardy periods and unexcused absences, good school discipline standards, and meet all school academic standards (2.5+ gpa). Work permits may be revoked at any time for not meeting these requirements.
